This case involves a 56 year old female who presented with vertical bone loss of the buccal plate of the implant abutment crown complex of her mandibular right first molar (Figure 1, 2). Figures 3 and 4 are showing full flap reflection with aggressive hand instrument debridement of the exposed implant body and residual bony defect along with detoxification utilizing citric acid. Figure 5 will show grafting of this defect utilizing a combination of mineralized allograft (MinerOss) along with rhBMP-2.
Next a Memlok collagen membrane was placed and secured over the grafted site (Figure 6) with primary closure obtained in Figure 7 utilizing 4-0 Vicryl suture. A two year clinical follow up is shown in Figure 8. Finally, Figures 9 and 10 show the before and two year follow up of that grafted site. Note the ossification on the buccal aspect of the implant body that has remained stable for 2+ years now.
